--Pap with HPV
--Pap without HPV
--Pap with reflex HPV if ASCUS
--Pap with reflex HPV if ASCUS/LSIL

Questions asked at order entry are required information:
--Collected Date/Time
--Clinical History/ICD Code
--Date of LMP
--History of Hormones, Pregnancy, Post-Partum Status, Post-Menopausal Status, Hysterectomy
--Specimen source
--Indication for Testing
Note:    ENDOCERVICAL BRUSHES AND BROOM-LIKE DEVICES SHOULD NOT BE USED ON PREGNANT PATIENTS. Refer to detailed collection instructions.

Do NOT send any body fluids (except urine) or tissues to the laboratory via the pneumatic tube system. They are to be physically "walked down" to the laboratory.
Processing Info:    If pathology/histology department is closed for the day, all other samples for cytology (body fluids, etc) go to the main lab for processing.
