A Wound Ostomy Continence Nurse (WOCN) is expertly trained to address the physical, emotional and social needs of patients with wounds, ostomies, stomas, urinary and fecal incontinence disorders. These nurses implement cost effective treatment plans to improve patient outcomes, reduce hospital stays, and provide valuable education to all members of the patient care team. Wound Ostomy Continence Nurses offer a wide range of services such as: providing patient follow-up care; patient, family and nursing staff education; creating individualized care plans, selecting optimal stoma sites preoperatively to promote post surgical independence in stoma care. WOC Nurses are key members in coordination of care across the health care continuum. They develop pressure ulcer and wound prevention programs. They support and guide patients and their families as they adapt to changes in lifestyle or management of chronic disease conditions such as crohns disease or ulcerative colitis. They manage bowel disorders and urine disorders.
