Heart failure, sometimes called congestive heart failure, is a chronic (long-term) condition caused by the inability of the heart to pump as much blood as the body needs. Heart failure becomes more common with age and causes symptoms such as shortness of breath and a build-up of fluid in the feet and legs. Heart failure is one of the most common reasons for hospitalization. A healthy lifestyle and proper medications can reduce the effects of heart failure and the need for future hospitalization. These indicators examine how well we manage the care of patients hospitalized for heart failure.



Written discharge instructions should include a list of all medications,when to follow up with physician, symtoms to call for, diet to follow, monitoring your weight, and recommended activity. This indicator shows how many patients recieved written instructions for all items.
LVF (Left ventricular function) assessment checks how the left chamber of the heart is pumping. This can show how serious the heart failure is.
ACE (angiotension converting enzyme) and ARB ( angiotension receptor blocker) are type of medicines used to treat a decreased function of the heart .
Smoking is linked to heart disease. Quitting smoking is very important in treating heart conditions.
