Financial Assistance

If you do not have healthcare insurance, you may be eligible for one of our free care programs. However, before you are considered for free care, we must know if you are eligible for MaineCare (Medicaid). Please contact your local Department of Human Services office to apply for MaineCare. 

To be considered for free care at CMMF, please download the CMMF Free Care form, complete it, and gather the following items:

• Proof of income for the last 13 weeks. If you are receiving a government check, we will need a copy of your yearly benefits letter. 

• If you had no income for the last 13 weeks, please complete the Income Statement Form and sign the document before a Notary Public. 

• Copy of your MaineCare denial letter.

• If your balance exceeds $5,000, or if you are self-employed, we will need a copy of your most recent federal income tax return.

CMMF is unable to make a determination without all necessary information.
