CMMC Ambulatory Diabetes Education and Follow-up (ADEF) |
Welcome to the Diabetes Education & Nutrition Center.
We are here to help you learn to care for your diabetes. Here is a brief overview of the services we offer:
Your doctor must refer you to the program.
The program includes:
- One-on-one visit with the nurse educator
- One-on-one visit with registered dietitian
Followed by two three-hour sessions:
- Session 1: General overview of diabetes, types of diabetes, high and low blood sugars, monitoring. Please bring your meter and medications.
- Session 2: Meal planning, carbohydrates, label reading, lab tests (cholesterol, triglycerides, and A1c), complications, resources and motivation, and how exercise, stress, and travel can affect blood sugars.
Classes are offered on a monthly basis as a one-day program from 9 a.m. to 4 p.m. or as four separate two-hour sessions. Call 795-2735 for a schedule of upcoming classes.
Follow-up visits to continue teaching and reinforcement are scheduled at one month, three months, six months, and one year.
Please note:
- Sessions are limited to 10 people with diabetes. Please call 795-2735 to reserve your space.
- In case of inclement weather, call 795-2735 or 795-0111 for changes in class schedule.
- There is a fee for the program and we encourage you to check with your insurance company to see if the fee is covered. Many insurance companies cover some or all of the program cost.
