As part of the Medical Center, comprehensive treatment by staff of medical and surgical subspecialties and advanced diagnostic services are immediately
available. All patients on the unit share the close collaboration of professionals in a broad range of rehabilitative disciplines, all working toward the common goal of helping patients regain their independence.

Our Clinical Care Coordinator works closely with a patient's acute-care hospital team to be assured that recommendations from this team follow the patient to the acute Rehab Unit. The rehabilitation process begins by involving patients and family members by having them participate in setting the focus of their rehabilitation stay and having them prepare for their discharge.

As a team, the focus is on what the patient can do, their strengths and maximizing independence. This team approach is the hallmark of the Physical Rehabilitation Unit at CMMC. Therapists and rehabilitation nursing will provide therapeutic programming for:

  • Regaining movement, strength and endurance
  • Assistance with mobility ( ability to get out of bed, to walk)
  • Regaining ability to care for self with tasks such as : feeding, bathing and dressing
  • When indicated, utilizing specialized equipment and dealing with architectural barriers outside of the hospital environment
  • Training Family members to provide assistance once a patient returns home.
  • Communication Deficits

  • Learning to deal with the disability beyong the rehab process
  • When indicated, dealing with swallowing adaptations and planning
