The team helps patients and families identify needs for outpatient or home health therapy services. The case manager plays a key role in assisting you to arrange services that you may require based on your transportation needs and your preference of service sites.

Central Maine Medical Family has options for you to consider if you need outpatient services. Our outpatient therapists work closely with the OICM Team to ensure a safe and accurate continuation of your rehabilitation plan and will provide timely updates about your progress to your surgeon.
